Abstract [sv]

Bakgrund: Ambulanssjukvården har utvecklats från en transporttjänst till en avancerad medicinsk enhet. Allmänhetens kunskap om att välja rätt vårdnivå är central för en effektiv vårdkedja och god tillgänglighet för de mest kritiskt sjuka patienterna. Problemformulering: Okunskap om mest lämplig vårdnivå leder till ökad belastning på 112 vid icke-akuta tillstånd. Samtidigt riskerar livshotande tillstånd att feltolkas och handläggas med fördröjning, vilket hotar patientsäkerheten. Det saknas studier som specifikt jämför allmänheten med vårdpersonal i att bedöma lämplig vårdnivå i samband med allvarliga och mindre allvarliga insjuknande. Syfte: Studien syftar till att undersöka allmänhetens kunskap om den mest lämpliga vårdinstansen vid olika typer av insjuknande som kräver bedömning och eventuell vård. Finns det en skillnad mellan vårdpersonal och allmänheten avseende synen på val av vårdnivå? Metod: En kvantitativ tvärsnittsstudie utfördes via en digital enkät som bestod av 17 olika patientfall. Svaren ställdes mot en expertbedömning av en akutläkare. Urvalet bestod av 177 respondenter, varav 92 hade yrkeserfarenhet inom vård och 85 tillhörde allmänheten. Data analyserades med Mann-Whitney U-test, chi-två-test samt z-test. Resultat: Vårdpersonal presterade signifikant bättre än allmänheten (p <0,001) med en högre andel rätta svar. Allmänheten tenderade att välja 1177 vid tidskritiska tillstånd som lungemboli och graviditetskomplikationer. Vid skador som inte var livshotande, exempelvis barn med fraktur, valdes ambulans i högre grad än vad expertbedömningen rekommenderade. Slutsats: Yrkeserfarenhet från vård och omsorg stärker förmågan att välja rätt vårdnivå, och kunskapsluckor synes föreligga hos allmänheten gällande val av vårdnivå vid olika typer av insjuknande. Resultatet indikerar på ett möjligt behov av specifika utbildningsinsatser för att öka allmänhetens kunskap, stärka patientsäkerheten och säkerställa ett effektivt resursutnyttjande inom ambulanssjukvården.

Abstract [en]

Abstract Background: Ambulance services have evolved from a transport service to an advanced medical unit. The general public's ability to choose the correct level of care for different types of onsets of illness is central to an efficient care chain including a high accessibility for the most critically ill patients. Problem Statement: A lack of knowledge regarding the most appropriate level of care leads to an increased burden on the emergency number 112 for non-emergency conditions. At the same time, life-threatening conditions risk being misinterpreted and undergoing delayed treatment, which threatens patient safety. There is a lack of studies specifically comparing the general public with that of healthcare professionals regarding the ability to choose the most appropriate level of care in various types of illness. Aim: The study aims to investigate the general public's knowledge of the most appropriate healthcare facility for various types of illnesses (medical conditions or accidents) requiring assessment and potential care. Is there any difference between healthcare professionals and the general public regarding their views on the choice of care level? Method: A quantitative crosssectional study was conducted via a digital survey consisting of 17 different patient cases. The responses were compared against an expert assessment by an emergency physician. The sample consisted of 177 respondents, of whom 92 had professional experience in healthcare and 85 belonged to the general public. Data were analyzed using the Mann-Whitney U-test, chi-square test, and z-test. Results: Healthcare professionals performed significantly better than the general public (p <0.001) with a higher proportion of correct answers. The general public tended to choose 1177 (the national healthcare advisory service) for time-critical conditions such as pulmonary embolism and pregnancy complications. For injuries that were not life-threatening, such as children with fractures, were ambulances chosen more often than recommended by the expert assessment. Conclusion: Professional experience from healthcare strengthens the ability to choose the correct level of care, and significant knowledge gaps appear to exist among the general public regarding various types of conditions. The results suggest the possible need for specific educational interventions to increase the public's knowledge, which may strengthen patient safety, and ensure efficient resource utilization within the ambulance services.
