Abstract [sv]

Bakgrund och problemformulering: Trauma är en av de vanligaste dödsorsakerna globalt. Inom ambulanssjukvården i Sverige utgör trauma en betydande del av uppdragen. I Sverige är däremot allvarliga trauman ovanliga, vilket kan leda till att vårdpersonal känner osäkerhet vid bedömning. För ökad patientsäkerhet inom traumavården utvecklades och implementerades de Nationella traumalarmskriterierna i Sverige. Trots att de nationella traumalarmskriterierna finns, föreligger oklarheter kring vilka patienter som omfattats av kriterierna. Syfte: Att beskriva förekomsten och karakteristiska hos prehospitala traumapatienter som har omfattats av de nationella traumalarmskriterierna samt att redogöra för den prehospitala bedömningen och det fortsatta vårdförloppet efter ankomst till sjukhus. Metod: En kvantitativ retrospektiv kohortstudie där patientjournaler granskats. Datamaterialet samlades in under hela 2019 från en region i sydvästra Sverige. Resultat: Studien inkluderade 380 patienter. Majoriteten var män (63,2 %) och bilolycka framkom som den vanligaste olyckshändelsen (52,6 %). Primärbedömning utfördes på 90,8 % av patienterna och merparten (84,7 %) transporterades med ambulans till en akutmottagning (83,9 %). En tredjedel (36,8 %) hade inläggningsbehov, 9,5% behövde IVA-vård och 0,8 % avled inom 30 dagar. Resultaten visar en hög övertriagering (87,5 %) och en hög undertriagering (74,1 %). Slutsats: Det anses svårt för ambulanspersonal att bedöma allvarlighetsgrad vid trauman samt tilldela patienter rätt ESS kod. De nationella traumalarmskriterierna anses vara komplexa och inte anpassade efter alla åldrar. Mer forskning inom området anses relevant samt eventuell modifiering och tydliggöranden gällande kriterierna för att öka träffsäkerheten. Det finns även utbildningsbehov för prehospitalpersonal gällande bedömning av allvarliga trauman.

Abstract [en]

Background and problem statement: Trauma is a leading cause of death worldwide and represents a substantial proportion of ambulance care assignments in Sweden. Because severe trauma cases are relatively rare in Sweden, healthcare professionals may experience uncertainty about patient assessments. To increase patient safety in trauma care, the Swedish national trauma team activation criteria were developed and implemented. Despite the implementation of these criteria in Sweden, there is still ambiguity and uncertainty regarding which patients are covered by the criteria. Aim: To describe the occurrence and characteristics of prehospital trauma patients who have been covered by the national trauma team activation criteria and to explain the prehospital assessment and the subsequent course of care after arrival at the hospital. Method: A quantitative retrospective cohort study in which patient records have been studied. The data material was collected during 2019 from a region in southwestern Sweden. Results: The study included 380 patients. The majority were men (63.2 %) and car accidents were the most common accident (52.6 %). The primary assessment was performed on 90.8 % of the patients and the majority (84.7 %) were transported by ambulance to an emergency department (83.9 %). One third (36.8 %) required hospitalization, 9.5 % needed intensive care and 0.8 % died within 30 days. The results also show a high overtriage (87,5 %) but also a high undertriage (74,1 %). Conclusion: It seems difficult for ambulance professionals to assess severe traumas and assign the correct ESS code. The National Trauma Triage Criteria seems complex and not modified to all ages. Further research in the area is needed, as well as possible modification and clarification of the criteria to increase its accuracy. In addition to this, there is also a need for training for prehospital personnel regarding the assessment of severe trauma.
