Abstract [sv]

Bakgrund och problemformulering: Anafylaxi är ett akut och potentiellt livshotande tillstånd som kräver snabb behandling med adrenalin. Trots tydliga riktlinjer visar forskning att prehospital vårdpersonal ofta har svårigheter att identifiera tillståndet och administrera korrekt dos i tid. Syfte: Syftet med den systematiska litteraturöversikten var att kartlägga vilka faktorer som påverkar ambulanssjuksköterskans beredskap för anafylaxi i prehospital vård. Metod: En systematisk litteraturöversikt genomfördes med fokus på prehospital behandling av anafylaxi. Databassökning utfördes i Scopus AI, PubMed och Cinahl. Sjutton originalartiklar publicerade mellan 2004 och 2026 inkluderades efter kvalitetsgranskning. Resultat: Fyra huvudteman identifierades: identifiering och bedömning av anafylaxi, hur prehospitalt givet adrenalin påverkar tiden från symtom till adrenalin, utbildningsinsatser och simuleringsmetoder samt organisatoriska och kliniska förutsättningar för adrenalinadministrering. Prehospitalt givet adrenalin förkortade tiden från symtomdebut till behandling och minskade risken för bifasiska reaktioner. Faktorer som utbildning, utrustning, aktör och kontext påverkade hur snabbt och korrekt adrenalin administrerades. Slutsats: Tidig identifiering och korrekt administrering av adrenalin är avgörande för prognosen vid anafylaxi. Förbättrad utbildning, tillgång till autoinjektor och ökad följsamhet till riktlinjer kan stärka patientsäkerheten och minska lidandet.

Abstract [en]

Background and problem statement: Anaphylaxis is an acute and potentially life threatening condition that requires prompt treatment with adrenaline. Despite clear clinical guidelines, research shows that prehospital healthcare personnel often experience difficulties in identifying the condition and administering the correct dose in time. Aim: The aim of this systematic literature review was to map the factors influencing ambulance nurses’ preparedness for anaphylaxis in prehospital care. Method: A systematic literature review was conducted focusing on prehospital management of anaphylaxis. Database searches were performed in Scopus AI, PubMed and Cinahl. Seventeen original articles published between 2004 and 2026, were included following quality appraisal. The results were graded for evidence strength using a four level scale from very low to high. Results: Four main themes were identified: identification and assessment of anaphylaxis; how prehospital administration of adrenaline affects the time from symptom onset to treatment; educational interventions and simulation methods; and organisational and clinical conditions for adrenaline administration. Prehospital administration of adrenaline shortened the time from symptom onset to treatment and reduced the risk of biphasic reactions. Factors such as education, equipment, personnel and context influenced how quickly and accurately adrenaline was administered. Conclusion: The findings indicate that early identification and correct administration of adrenaline are crucial for the prognosis of anaphylaxis. Improved education, access to auto injectors and greater adherence to guidelines may strengthen patient safety and reduce suffering in prehospital care.
