Practicing for the Unexpected : The Significance of Simulation-Based Education for Emergency Nurses' Competence and Preparedness (English)
Abstract [en]

Disasters and crises are becoming more frequent worldwide. The capacity of the emergency department is challenged during an unforeseen event, and emergency nurses experience a lack of preparedness, which may be explained by insufficient education. A mass casualty incident involves a sudden major event with many injured individuals that creates an imbalance between needs and existing healthcare resources. When this imbalance occurs, ethical principles may conflict with one another and ethical dilemmas may arise. Through simulation-based training, the emergency nurse is given an opportunity to practice without jeopardizing patient safety. The aim is to investigate the significance of simulation-based training for the emergency nurse’s preparedness and competence in mass casualty incidents. An integrative literature review was used as a method, including qualitative and quantitative designs. Fourteen articles were analyzed. The results revealed four themes: Professional development, Action capability, Education and Collaboration. Eight subthemes were identified: Knowledge and Skills, Experience, Awareness, Ability to Act, Skills Training, Motivation and Attitudes, Leadership, Roles and Teamwork. Simulation-based training provides a positive experience, reflects reality, and highlights potential problems that may arise. Simulation-based training can be used as a complement to traditional education to strengthen the emergency nurse’s competence and preparedness, as well as to clarify the different professional roles.

Katastrofer och kriser blir mer frekventa världen över. Akutmottagningens kapacitet utmanas vid en oförutsedd händelse och akutsjuksköterskorna upplever en bristande beredskapsnivå vilket kan förklaras av en otillräcklig utbildning. En masskadehändelse innebär att en plötsligt stor händelse med många skadade skapar en obalans mellan behov och befintliga resurser inom sjukvården. När denna obalans uppstår så kan de etiska principerna ställas i konflikt mot varandra och etiska dilemman kan uppstå. Genom simuleringsbaserad träning ges akutsjuksköterskan en möjlighet att öva utan att äventyra patientsäkerheten. Syftet är att undersöka betydelsen av simuleringsbaserad träning för akutsjuksköterskans beredskap och kompetens vid masskadehändelse. En integrativ litteraturöversikt har använts som metod som inkluderar kvalitativa och kvantitativa design. 14 artiklar analyserades. I resultatet framkom fyra teman: Professionell utveckling, Handlingsförmåga, Utbildning och Samverkan. Åtta subteman: Kunskap och färdighet, Erfarenhet, Medvetenhet, Att kunna agera, Färdighetsträning, Motivation och attityder, Ledarskap samt Roller och teamarbete. Simuleringsbaserad träning ger en positiv upplevelse, återspeglar verkligheten och vilka problem som kan uppstå. Simuleringsbaserad träning kan användas som ett komplement till traditionell utbildning för att stärka akutsjuksköterskans kompetens och beredskap samt skapa tydlighet av de olika yrkesrollerna.
