Abstract [sv]

Våren 2020 startade Covid-19 pandemin och satte stor press på sjukvården. Kritiskt sjuka patienter sökte vård på akutmottagningarna med en sjukdom sjuksköterskorna visste lite om. Omvårdnaden kring sjukdomen var främmande och skapade press hos sjuksköterskor som trots rädsla och kunskapsbrist förväntades leverera god vård. Detta påverkade sjuksköterskor yrkesmässigt och individuellt. Syftet med litteraturöversikten är att beskriva sjuksköterskors upplevelse att arbeta på akutmottagningen under Covid-19 pandemin. Metodvalet var allmän litteraturöversikt av kvalitativ forskning med induktiv ansats som gav 10 vetenskapliga artiklar till resultatet via databaserna MEDLINE och CHINAL. Resultatet genererade 3 huvudteman, -Arbetsmiljö och organisation, -Emotionell belastning och -Hantering och utveckling. Arbetsmiljö och organisation beskriver hur arbetsbelastningen upplevdes på akutmottagningar och vilket stöd som sjuksköterskorna fick av organisationen. Vilken påverkan skyddsutrustningen hade på omvårdnaden och hur riktlinjer erfors och vilka konsekvenserna blev för sjuksköterskorna och patienterna. Temat emotionell belastning beskriver sjuksköterskornas stress, press och utmaningarna de erfor. Rädslan för Covid-19 och skuldkänslor att inte utöva professionell omvårdnad. Sista huvudtemat hantering och utveckling beskriver copingstrategier och hur sjuksköterskorna växte genom krisen och lärde sig av situationen. En slutsats är att sjuksköterskorna under covid-19-pandemin upplevde hög stress på akutmottagningarna, då vårdandet i större utsträckning kom att fokusera på symtomlindring snarare än på helhetsinriktad omvårdnad. De beskriver att den största anledningen till att de klarade av Covid-19 pandemin var goda relationer och samtal med kollegor. Sjukvårdsorganisationerna har stort ansvar att jobba proaktiv för framtida pandemier och kriser. Sjuksköterskor ska inte behöva utsättas för oproportionerliga påfrestningar yrkesmässigt och individuellt för att främja hållbara arbetsplatser och bevara kunskap.

Abstract [en]

In the spring of 2020, the COVID-19 pandemic emerged and placed significant pressure on healthcare systems worldwide. Critically ill patients sought care in emergency departments with a disease about which nurses had limited knowledge. Nursing care related to the disease was unfamiliar and created pressure on nurses who, despite fear and lack of knowledge, were expected to deliver high-quality care. This situation affected nurses both professionally and individually. The aim of this literature review was to describe nurses’ experiences of working in the emergency department during the COVID-19 pandemic. The methodological approach was a general literature review of qualitative research with an inductive design, which resulted in ten scientific articles included in the findings, retrieved through the databases MEDLINE and CINAHL. The results generated three main themes: Work environment and organization, Emotional burden, and Coping and development. The theme Work environment and organization describes how workload was experienced in emergency departments and the level of support nurses received from their organizations. It also addresses the impact of personal protective equipment on nursing care, how guidelines were experienced, and the consequences for both nurses and patients. The theme Emotional burden describes nurses’ stress, pressure, and the challenges they experienced, including fear of COVID-19 and feelings of guilt related to being unable to provide professional nursing care. The final theme, Coping and development, describes coping strategies and how nurses grew through the crisis and learned from the situation. One conclusion is that nurses working in emergency departments during the COVID-19 pandemic experienced high levels of stress due to changes in care delivery, resulting in nursing care becoming more task-oriented rather than caring in nature. Nurses described supportive relationships and communication with colleagues as the most important factors enabling them to cope with the pandemic. Healthcare organizations have a significant responsibility to work proactively to prepare for future pandemics and crises. Nurses should not be exposed to disproportionate professional and individual strain in order to promote sustainable workplaces and preserve nursing competence.
