    Abstract [en]

    Multidrug resistance (MDR) in tumor cell lines is frequently correlated with amplification of one or more mdr genes. Usually the amplified domain also includes several neighboring genes. Using pulsed-field gel electrophoresis, we have established a restriction map covering approximately 2,200 kb in the drug-sensitive mouse tumor cell line TC13K. The mapped region is located on mouse chromosome 5 and includes the three mdr genes, the gene for the calcium-binding sorcin protein, and a gene with unknown function designated class 5. Long-range maps of the amplified DNA sequences in five of six MDR sublines that had been independently derived from TC13K generally displayed the same pattern as did the parental cell line. All six MDR sublines exhibited numerous double minutes, and one of them displayed a homogeneously staining region in a subpopulation. Large circular molecules, most likely identical to one chromatid of the double minutes, were detected in four of the sublines by linearization with gamma irradiation. The size of the circles was about 2,500 kb, which correlated to a single unit of the amplified domain. We therefore propose that in four independent instances of MDR development, a single unit of about 2,500 kb has been amplified in the form of circular DNA molecules. The restriction enzyme map of the amplified unit is unchanged compared with that of the parental cell line, whereas the joining sites of the circular DNA molecules are not identical but are in the same region.

    Many different drugs may be used in selecting cells for multidrug resistance (MDR). Enhanced expression and/or gene amplification is known to cause overproduction of membrane-bound 170,000 P-glycoproteins, responsible for the MDR. In rodents, the P-glycoproteins are encoded by a small gene family: mdr 1a, mdr 1b, and mdr2. To evaluate the relationship between the pattern of MDR and the selecting drug, nine MDR sublines were independently selected from a sensitive mouse tumor cell line, SEWATC13K, using three different drugs. Each MDR subline displayed amplification of one or more of the three mdr genes, but only one, mdr 1a, was consistently overexpressed. Thus, our results indicate that the pattern of mdr gene amplification and overexpression is independent of the selective agent. Furthermore, in four of the MDR sublines, where all three mdr genes had been originally amplified, pulsed field gel electrophoresis (PFGE) revealed that amplification of mdr 1a, only, was a second step of gene amplification. In addition, the gene for the calcium-binding protein, sorcin, was coamplified in eight of the nine MDR sublines. The sorcin gene was overexpressed in seven of these eight sublines. Finally, hybridizations with a probe homologous with a putative region of RFLP (restriction fragment length polymorphism), indicated that the amplified sequences originate from one or the other of the two homologous chromosomes with no preference.

    The aim of this study is to detect similarities and differences in the gene amplification process, when MDR cells develop after selection in different drugs. For this purpose, nine independent MDR sublines of the murine SEWA TC13K cell line were established by stepwise selection in actinomycinD( Al,A2,A4),colcemid(Cl,C2,C3),or vincristine (Vl, V3, V4). All MDR sublines displayed numerous DMs. DNA from all lines was digested with EcoRl and the infrequently cutting enzyme SfiI. DNA-fragments from the SfiI-digests were separated by orthogonal field alterating gel electrophoresis (OFAGE). Hybridizations were made with two probes: (1) cp28, a Chinese hamster P-glycoprotein cDNA sequence, kindly supplied by Dr Alexander Van der Bliek, The Netherlands Cancer Institute in Amsterdam; (2) Ie7, a genomic clone from a DM mini-library (Stihl et al., Hereditas 106:97, 1987). The hybridization patterns of the Sfildigests were very similar for both probes, while the patterns of the EcoRI-digests differed considerably. No drugspecific amplification pattern was revealed. The Ie7 probe did not cross-hybridize to the cp28 cDNA-sequence. However, low stringency hybridization of Ie7 to a 4.5 kb mRNA (hamster) has been reported (Diddens et al., Int. J. Cancer 40:635, 1987). Still, the similarities in the hybridization patterns suggest that Ie7 DNA-sequence is located close to the cp28 DNA-sequence, perhaps as a part of an intron. In the SfiI digest the cp28 probe hybridized to seven DNA-fragments (six of which were detected also with Ie7) that were amplified in most of the lines. The same DNAfragments were present but not amplified in the control line TC13K. The presence of several hybridizing fragments also in the control line is probably due to homology within the P-glycoprotein gene family. Other amplified sequences were unique to each cell line and are presumably a result of so-called novel joints. The large number of new hybridizing fragments thus reflects a recombinational process during amplicon formation. The presence of specific cytogenetic markers in some of the cell lines indicated that the MDR cells were of monoclonal origin. Therefore, it is possible that each line contains a homogeneous population of DMs, each contributing the same complex pattern of novel fragments in the SfiI analysis. Another explanation of this complexity would be thai different amplicons (with different novel joints) are located on different DMs in a heterogeneous DM population.

    Människor som är sjuka/skadade söker sig dagligen till akutmottagningar i hopp om att få hjälp. Där arbetar det flera olika personalkategorier som ansvarar för att dessa människor skall få bästa möjliga vård. Det har kommit in rapporter om att patienter inte alltid har upplevt att de blivit mottagna på ett värdigt sätt. Denna litteraturstudies syfte är att beskriva patienters upplevelse av mötet med vårdpersonal på akutmottagningar. Metoden är en litteraturstudie där sju kvalitativa artiklar analyserats enligt Evans (2003) kvalitativa analysmodell. Resultatet av denna studie presenteras utifrån fyra teman som är Att inte få veta, Upplevelse av ensamhet, Vårdarens brist på empati och Känsla av trygghet. Två teman presenteras utifrån subteman. Det visar sig att patienter har både dåliga, förskräckliga och bra upplevelser av mötet med vårdpersonalen på akutmottagningar. Att inte få veta handlar om informationens betydelse för patienten. Upplevelse av ensamhet handlar om att inte bli sedd, att bli övergiven. Vårdarens brist på empati handlar om hur patienterna upplever vårdarna osympatiska. Känsla av trygghet beskriver, att få kunskap, att befinna sig på akutmottagningen och att bli betraktad som en individ..

    Egenvård har en central roll i behandlingen av diabetes typ 2. Egenvård innebär främst livsstilsförändringar i form av ökad fysisk aktivitet och kostförändringar och kan i vissa fall ersätta behandling med läkemedel. Att ändra sin livsstil är ofta svårt och innebär för många individer en livsomvälvande process. Sjuksköterskan har en viktig roll i att informera och motivera patienten i denna process så att patienten kan uppnå en god hälsa och känna välbefinnande trots sin sjukdom. Syftet med denna litteraturöversikt är att ur ett patientperspektiv undersöka vad som främjar och hindrar egenvård hos patienter över 45 år med diabetes typ 2. Data analyserades med hjälp av Fribergs modell (Friberg 2017, ss. 148-150) för litteraturstudier.

    I studiens resultat presenteras aspekter som främjar och hindrar egenvård som två huvudkategorier och vid dataanalysen identifierades tre underkategorier till respektive huvudkategori. Dessa behandlade hur familj, vårdpersonal och andra sociala relationer både främjade och hindrade individens egenvård. Betydelsen av kunskap framgick också liksom värdet av motivation och egna strategier. I diskussionen belyses kunskapens betydelse för en fungerande egenvård samt hur anhöriga och sociala relationer kan påverka individens upplevelse av sjukdomen och förmåga till egenvård. Med stöd från anhöriga och ökad kunskap om sjukdomen och egenvårdens betydelse skapas förutsättningar för att patienten trots sjukdom kan uppleva hälsa och välbefinnande.

    Bakgrund och problemformulering: Efter att vi båda har arbetat inom hälso- och sjukvård i över 10 år som fysioterapeuter har vi ofta stött på och blivit medvetna om att många sjuksköterskor uppger att dom har fått arbetsrelaterad ohälsa på grund av ökad stress i sitt yrke som gett varierande psykiska och somatiska symtom med påföljande sjukskrivningar som följd. Det har i ett flertal fall lett till att sjuksköterskan inte har kunnat fortsätta arbeta som tidigare och i vissa fall fått byta karriär eller ofrivilligt gå ner i arbetstid för att orka med. Vi har uppmärksammat att det gäller både sjuksköterskor som arbetat många år i yrket och de som endast arbetat i ett till ett par år.

    Syfte: Undersöka bakomliggande faktorer till utveckling av fatigue hos sjuksköterskor.

    Metod: Litteraturstudie enligt Fribergs (2017) modell.

    Resultat: Det är ett flertal orsaker som ligger bakom utvecklingen av fatigue. Organisationen och ledningen har visats ha betydelse för hur väl individen hanterar sitt arbete vad gäller logistik, rutiner, schemaläggning och bemanning. Den psykosociala miljön är viktig för att sjuksköterskan ska känna sig trygg och få stöd och uppleva trivsel i sitt arbete. En allt tyngre omvårdnad på grund av äldre och mer sjuka patienter på vårdavdelningarna är en annan bidragande orsak. Individens personliga egenskaper såsom erfarenhet, ålder och den privata familjesituationen är viktiga för hur en hög arbetsbelastning kan hanteras.

    Diskussion: Det ligger i både arbetsgivarens och individens intresse att i tid upptäcka när arbetssituationen inte längre är hälsosam. Arbetsgivaren kan skapa kontrollfunktioner för att upptäcka arbetstagares nivå av stress och behov av återhämtning innan det går så långt att sjuksköterskan måste sjukskriva sig.

    Multimorbidity, a condition common among olderadults, may be regarded as a failure of a complex system.The aim of this study was to describe the corecomponents in health and social care management forolder adults with multimorbidity. A cross-sectionaldesign included two methods: individual interviews andgroup discussions. A total of 105 participants includedolder adults with multimorbidity and their relatives,care staff and healthcare policymakers. Data were analysedusing content analysis. The results show thatseven core components comprise a multiperspective 

    view of health and social care management for olderadults with multimorbidity: political steering, leadership,cooperation, competence, support for relatives, availabilityand continuity. Steps should be taken to ensure thatevery older adult with multimorbidity has a treatmentplan according to a multiperspective view to preventfragmentation of their health care. This study providesrelevant evidence developing a multiperspective modelof health and social care management for older adultswith multimorbidity.

    Abstract [en]

    Healthcare environment in intensive care units (ICUs) is proved to have physiological effects and the ability to facilitate health and wellbeing as well as worsen the already critical ill patients’ status. The work environment for critical care nurses is reported unhealthy and research has implicated high risk for developing burnout due to occupational matters. The technological progress has evolved enormously during the last decades, but the design of the ICU has not developed in the same pace. Evidence-based design (EBD) is a concept integrating knowledge from various research disciplines and its application to healing environments.

    The aim was to explore nursing staff’s experiences of working in an evidence-based designed ICU patient room.

    A patient room was renovated and furnished, according to the principles of EBD. The room was equipped with acoustic panels in the walls and ceiling and a cyclic light system was installed. In addition prototype pendulums were installed, equipped with lights, electrical sockets and medical gas supplies. The room had a window and door leading onto a patio with furniture and plants, which were accessible to patients and their relatives. Interviews with eight critical care nurses and five assistant nurses were conducted and analysed by qualitative content analysis.

    The result of the study showed that an evidence-based designed ICU patient room stimulated alertness and promoted wellbeing among the participants, cultivated their caring activities. The prototype design of the medical and technical equipment challenged their nursing actions.

    The room had been refurbished in order to create a healing environment. This study found that the new environment had a great impact on the participants’ wellbeing and caring behavior and the need for flexibility in patient rooms. At a time when sick leave is increasing and turnover in nurses is high, these findings showed the importance of design of ICUs.

    Medical technology has progressed tremendously over the last few decades, but the same development cannot be seen in the design of these intensive care unit environments. Authors report results of a study of evidence-based room design, emphasizing the impact on conveying a caring attitude to patients. Ten nonparticipant observations were conducted in patient rooms with 2 different designs, followed by interviews. The data were analyzed using a phenomenological-hermeneutical approach. The results did not reveal that it was obvious that redesigned spaces resulted in a more caring attitude. The meanings of caring displayed during nursing activities were interpreted by interpreting gazes. Some of the nursing staff had an instrumental gaze, interpreted as caring with a task-orientated approach, while others communicated their caring with an attentive and attuned gaze, where the needs of the patients regulated the working shift. The study findings indicated that caring may not be perceived when nurses use a task-oriented approach. However, when nurses practice a person-centered approach, using an attentive and attuned gaze, caring is conveyed. Caring in intensive care contexts needs to be assisted by a supportive environment design that cultivates the caring approach.

    INTRODUCTION: It has been known for centuries that environment in healthcare has an impact, but despite this, environment has been overshadowed by technological and medical progress, especially in intensive care. Evidence-based design is a concept concerning integrating knowledge from various research disciplines and its application to healing environments.

    OBJECTIVE: The aim was to explore the experiences of nursing staff of working in an evidence-based designed ICU patient room.

    METHOD: Interviews were carried out with eight critical care nurses and five assistant nurses and then subjected to qualitative content analysis.

    FINDINGS: The experience of working in an evidence-based designed intensive care unit patient room was that the room stimulates alertness and promotes wellbeing in the nursing staff, fostering their caring activities but also that the interior design of the medical and technical equipment challenges nursing actions.

    CONCLUSIONS: The room explored in this study had been rebuilt in order to create and evaluate a healing environment. This study showed that the new environment had a great impact on the caring staffs' wellbeing and their caring behaviour. At a time when turnover in nurses is high and sick leave is increasing, these findings show the importance of interior design of intensive care units.

    BACKGROUND: The importance of the clinical learning environment in nurse education has gained increasing attention over the last decades. However, there is a lack of research on the learning environment, its significance and meaning in specialist nurse education.

    OBJECTIVE: The objectives of the study were to investigate specialised nursing students' experiences of supervision during clinical practice and to compare students who were satisfied with the supervision with those who were dissatisfied with respect to a) organisation of supervision and number of preceptors, as well as time allocated by preceptors for b) supervision, c) reflection, d) discussion of intended learning outcomes, and e) assessments of students' performance by preceptors.

    DESIGN: This study used a cross-sectional design.

    SAMPLE AND SETTINGS: A convenience sample of specialised nursing students was derived from five Swedish universities in the years 2016 and 2017.

    METHODS: Data were collected using a questionnaire. Statistical analyses and a qualitative conventional content analysis were performed.

    RESULTS: While almost all specialised nursing students reported that there had been time for discussion on their performance assessment, almost half of the students reported not getting time for supervision, or time for reflections and discussions on intended learning outcomes with the preceptor. Students reporting having time allocated for supervision by preceptors were found to be more satisfied with supervision. It was described as important that the preceptor(s) acknowledged the students previous work experiences. Even though being a registered nurse, reflections and feedback were described as valuable for the students learning. Several preceptors were described as positive allowing a broader picture and different views regarding working as a specialist nurse.

    CONCLUSIONS: This study indicates that supervision, in terms of discussions and reflections, of specialised nursing students is significant for learning experiences and satisfaction during clinical placement.

    Abstract [en]

    BACKGROUND: Previous research points to challenges related to the home healthcare of older persons and to the complexity of communication. Although person-centred care has been advocated widely, there remains a need for in-depth knowledge on how to enable person-centred and supportive communication in the care of older persons.

    AIM: The aim of this study was to explore attributes of person-centred communication between nurses and older persons being cared for in their home.

    METHODS: A descriptive study with a qualitative approach was conducted. A data set from the COMHOME-study consisting of 77 audio-recorded home healthcare visits between registered nurses and older persons was analysed with a method for qualitative thematic analysis.

    RESULTS: The findings indicate that the attributes of person-centred communication comprise recognising, inviting and involving older persons. To facilitate this form of communication, attentiveness and responsiveness on the part of RNs seemed significant. Person-centred communication was facilitated when the RNs used verbal expressions to emphasise and acknowledge the older persons' views and were attentive to their emotions and expressions.

    CONCLUSION: The nurses' attentiveness and responsiveness seems important for person-centred communication with older persons. Communication skills are needed to recognise, invite and involve older persons in their care and to support their health and well-being. Implication for practice The importance of communication which facilitate a person-centred approach by nurses should be acknowledged when caring for older persons and included in education and training.

    Abstract [en]

    The quality of health care and patient safety in the operating room is a major concern for nurse anaesthetists. However, few studies have focused on the experiences of nurse anaesthetists’ and their contributions to safety in this setting. Therefore, this study aims to explore the content and frequency of incidents reported by nurse anaesthetists in the operating room and the risks involved in these incidents. A retrospective study with a descriptive design was conduct. Data were gathered concerning 220 incidents reported by nurse anaesthetists from 2012 to 2015 in operating rooms at a middle-sized hospital in Sweden. These were analysed with a method for qualitative and quantitative content analysis. The findings are presented in five categories: communication and teamwork; routines and guidelines; patient care; nurses’ work environment; devices, materials and technologies. In 184 (73%) of the incidents, there was either a risk of harm or there was an actual harm to patients or nurses. Of all incidents only 23 (10%) had harmed patients or nurses. Few of these incidents involved patient harm (= 6), while a greater number involved harm to nurses (= 17). The findings reveal lack of communication and interprofessional teamwork as the two most common areas for the reported incidents, followed by problems related to lack of compliance with guidelines and routines. The findings suggest that strategies are needed to improve these areas. Patient safety reporting systems may be important to identify risk in preventing patients and health care professionals from being harmed. In addition, the findings indicate that the nurses sought to prevent harm to patients rather than to themselves. Consequently, increased attention to the work environments of nurses, and most likely other professionals, in the operating room may be needed to prevent health care professionals from being harmed.

    Aim: The aim of this paper was to discuss how to understand and undertake thematic analysis based on descriptive phenomenology. Methodological principles to guide the process of analysis are offered grounded on phenomenological philosophy. This is further discussed in relation to how scientific rigour and validity can be achieved.

    Design: This is a discursive article on thematic analysis based on descriptive phenomenology.

    Results: This paper takes thematic analysis based on a descriptive phenomenological tradition forward and provides a useful description on how to undertake the analysis. Ontological and epistemological foundations of descriptive phenomenology are outlined. Methodological principles are explained to guide the process of analysis, as well as help to understand validity and rigour. Researchers and students in nursing and midwifery conducting qualitative research need comprehensible and valid methods to analyse the meaning of lived experiences and organize data in meaningful ways.

    Abstract [sv]

    Denna rapport visar att patienters rättigheter är otydliga när det gäller rätten till hälso- och sjukvård. Otydligheten kan leda till att hälso- och sjukvården framstår som rättsosäker. Det finns möjligheter att förbättra kvalitet, trygghet och patientsäkerhet, där anmälningar från patienter och närstående kan utgöra ett värdefullt underlag. En av rapportens slutsatser är att det finns ett behov av en mer patient- och personcentrerad hälso- och sjukvård.

    I denna rapport beskrivs resultaten från ett forskningsprojekt där anmälningar till patientnämnderna i Västra Götaland analyserats med fokus på mänskliga rättigheter. Ett strategiskt urval av 170 anmälningar som patienter eller närstående lämnat till patientnämnderna under år 2015 har här analyserats.

    Resultaten beskrivs i fyra delar. I den första delen presenteras beskrivande statistik för anmälningarna. Den andra delen beskriver innehållet i anmälningarna, vilket handlar om att inte ha tillgång till hälso- och sjukvård, brister i kontinuitet och uppföljning, felaktigheter eller vårdskador till följd av hälso- och sjukvård, brister i kommunikation och samverkan, bemötande, attityder och förhållningssätt i möten med hälso- och sjukvårdspersonal, samt hälso- och sjukvård som går emot patientens vilja. Därefter följer den del där anmälningarna analyserats utifrån ett patient- och mäniskorättsperspektiv. Här beskrivs fem teman som berör rätten till hälsa och patienters rättigheter vid ohälsa och sjukdom, rätten till hälso- och sjukvård och patienters rättigheter vid akuta tillstånd, rätten till god och säker hälso- och sjukvård och patienters säkerhet, rätten till värdighet och ett bra bemötande, samt hälso- och sjukvårdspersonalens rättigheter. I den fjärde och sista delen av resultaten har anmälningarna granskats utifrån ett normkritiskt och intersektionellt perspektiv. Här framträder vissa grupper av patienter som mer sårbara än andra, och den utsatthet och maktaspekter som framträder i analysen beskrivs.

    I anmälningarna till patientnämnderna framkommer situationer där patienters rättigheter brister i möten med hälso- och sjukvården, där det kan vara svårt för patienter att utkräva sin rätt. Anmälningarna synliggör patienters upplevelser av att kvalitet, trygghet och patientsäkerhet brister, och patientnämndernas verksamhet spelar en viktig roll i att synliggöra dessa brister för att kunna bidra till förbättringar inom hälso- och sjukvården. För att säkerställa de mänskliga rättigheterna och rätten till hälsa behöver hälso- och sjukvården anpassas utifrån invånares behov.

    Anmälningarna visar att det ibland finns brister i hälso- och sjukvårdens förmåga att utgå från en helhetssyn på patienten. Enskilda personer är sårbara i kontakt med hälso- och sjukvården, och patienters utsatthet och olika maktförhållanden mellan patienter och hälso-  och sjukvårdspersonal behöver hanteras ansvarsfullt för att inte skapa ojämlikheter eller leda till upplevelser av diskriminering. Ett professionellt bemötande kräver också kommunikativ kompetens och empati hos hälso- och sjukvårdspersonal. En slutsats av detta är att det finns behov av en mer patientcentrerad hälso- och sjukvård, som tydligare beaktar enskilda personers förutsättningar och behov, liksom organisatoriska förbättringar som främjar patientens vårdkedja och samverkan mellan vårdverksamheter.

    Child abuse recognition and the protection of children is a global concern. In Sweden, the role of the school nurse (SN) is to promote schoolchildren's health and development and to identify and prevent harm. The aim of this study was to describe Swedish SN experiences of suspecting, identifying, and reporting child abuse and to compare them with respect to (a) years of experience as SN, (b) age of SN, and (c) pupil population size. A descriptive design was used. Two-hundred and thirty-three SNs completed a survey detailing their experiences. Most SNs (96%) reported having suspected a child suffering from physical or psychological abuse. Approximately half of them reported occurrences of honor-related violence (54%) and of child sexual abuse (57%). SNs with less nursing experience reported significantly less recognition and reporting of child abuse. The findings indicate that experiences of child abuse are common. Thus, it is vital that SNs have the necessary competency and support to identify and report suspected child abuse.

    Aim The aim was to investigate student nurses' experiences of the clinical learning environment in relation to how the supervision was organized. Background The clinical environment plays an essential part in student nurses' learning. Even though different models for supervision have been previously set forth, it has been stressed that there is a need both of further empirical studies on the role of preceptorship in undergraduate nursing education and of studies comparing different models. Method A cross-sectional study with comparative design was carried out with a mixed method approach. Data were collected from student nurses in the final term of the nursing programme at three universities in Sweden by means of a questionnaire. Results In general the students had positive experiences of the clinical learning environment with respect to pedagogical atmosphere, leadership style of the ward manager, premises of nursing, supervisory relationship, and role of the nurse preceptor and nurse teacher. However, there were significant differences in their ratings of the supervisory relationship (p < 0.001) and the pedagogical atmosphere (p 0.025) depending on how the supervision was organized. Students who had the same preceptor all the time were more satisfied with the supervisory relationship than were those who had different preceptors each day. Students' comments on the supervision confirmed the significance of the preceptor and the supervisory relationship. Conclusion The organization of the supervision was of significance with regard to the pedagogical atmosphere and the students' relation to preceptors. Students with the same preceptor throughout were more positive concerning the supervisory relationship and the pedagogical atmosphere.

    Abstract [en]

    Coronary heart disease is a major cause of sudden death and morbidity in the developed world, as well as a cause of great suffering. Research within this area has primarily focused symptoms, risk factors and treatment. The aim of this paper was to explore women's experiences following a myocardial infarction (MI). Eight women were interviewed; the interviews were audiotaped and transcribed into text and analysed using a phenomenological approach. To explore the meaning that is experienced in the lived world of the patient a method of reflective lifeworld research, based upon phenomenological epistemology has been used. The results indicate that the body is vital for the women in their lifeworlds. After a MI the patient's natural and unreflective relationship with the body and the lived world is interrupted. Uncertainty about life and death as well as the body is experienced as a suffering in the women's lifeworlds. In relation to this, the women's existence is characterised by an uncertainty and a loss of context. It is through reconciliation with their bodies and their illnesses that the women can achieve a sense of well-being and harmony in life. In that process the women can re-establish a natural relationship with their bodies and lifeworlds.

    Relationships and sexuality following heart attack (MI) have been studied; nevertheless, little is known about the meaning of social support and relationships to health and well-being after an MI. To our knowledge, no qualitative studies have further investigated the phenomenon. In this study we explore the meaning of close relationships and sexuality to women's health and well-being following MI. Ten women were interviewed using a reflective lifeworld approach and phenomenological epistemology. The meaning of women's close relationships following an MI appears to be closely intertwined with their long-term health process; both health processes and the relationships are affected. Suffering after an MI can be compared to taking a fall in that close relationships can become a safety net. Close relationships and sexuality are integrated into their lived bodies, and in that way have profound influence in their lifeworld experiences. Not all close relationships are intimate; however, all close and meaningful relationships can provide power and strength to the women's health processes. At the same time, these relationships also appear to drain energy and cause suffering.

    AIM: To explore communicative challenges in encounters between nurse assistants and older persons during home care visits.

    BACKGROUND: The older population is increasing worldwide. Currently, there is a shift in care for older people from institutional care to home care. Providing home care in a person's home involves several challenges, including the complexity of communication.

    DESIGN: A descriptive observational design with a qualitative approach was used.

    METHODS: The data consisted of audio recordings of real-life encounters during home care visits between nurse assistants and older persons, collected in 2014. A hermeneutic phenomenological analysis was conducted.

    RESULTS: Communicative challenges were identified: (a) in situations where the older persons had a different view than the nurse assistants on the care task and its content; and (b) when unexpected actions or turns occurred in the communication. Challenges included older person's existential issues, fragility and worries and concerns, which often appeared to be only vaguely expressed and difficult to verbally detect and tackle. This engendered a risk of misinterpretation or ignorance of these challenges.

    CONCLUSION: The findings point to the importance of communication as the key to facilitate person-centred home care. Communication training should focus more on addressing needs and existential issues in older persons. Person-centred home care for older persons needs to be addressed at both an individual and an organizational level.

    The aim of this study was to illuminate the lived experiences of families where a child had survived 7 years from a diagnosis of childhood cancer. This article describes one part of an inductive and longitudinal research project that included 17 families. Four families whose child was diagnosed with cancer 7 years previously were interviewed using a hermeneutical phenomenological approach. The families lived experience was described in one essential theme, "Living an everyday life through the child's cancer trajectory," further illuminated in 3 related themes: "Leaving the disease behind yet feeling its presence," "Being the same yet always different," and "Feeling stronger yet vulnerable." The results suggest that family members feel vulnerable even if a long period of time has passed since completion of treatment. To varied degrees they still may need support. When moving forward in life, the family members are helped if they can reconcile their memories and experiences derived from the childhood cancer trajectory.

    OBJECTIVE: This study aims to a) explore to what extent older persons express emotional cues and concerns during home care visits; b) describe what cues and concerns these older persons expressed, and c) explore who initiated these cues and concerns.

    METHODS: A descriptive and cross-sectional study was conducted. Data consisted of 188 audio recorded home care visits with older persons and registered nurses or nurse assistants, coded with the Verona coding definitions on emotional sequences (VR-CoDES).

    RESULTS: Emotional expressions of cues and concerns occurred in 95 (51%) of the 188 recorded home care visits. Most frequent were implicit expressions of cues (n=292) rather than explicit concerns (n=24). Utterances with hints to hidden concerns (63,9%, n=202) were most prevalent, followed by vague or unspecific expressions of emotional worries (15,8%, n=50). Most of these were elicited by the nursing staff (63%, n=200).

    CONCLUSION: Emotional needs expressed by the older persons receiving home care were mainly communicated implicitly. To be attentive to such vaguely expressed emotions may demand nursing staff to be sensitive and open.

    PRACTICE IMPLICATIONS: The VR-CoDES can be applied on audio recorded home care visits to analyse verbal and emotional communication, and may allow comparative research.

    Simulation has become a widely used and established pedagogy for teaching clinical nursing skills. Nevertheless, the evidence in favour of this pedagogical approach is weak, and more knowledge is needed in support of its use. The aim of this study was (a) to explore the experiences of undergraduate nursing students when examining knowledge, skills and competences in clinical simulation laboratories with high-fidelity patient simulators and (b) to analyse these students' learning experiences during the examination. A phenomenological approach was used, and qualitative interviews were conducted among 23 second-year undergraduate nursing students-17 women and 6 men. The findings revealed that, irrespective of whether they passed or failed the examination, it was experienced as a valuable assessment of the students' knowledge and skills. Even if the students felt that the examination was challenging, they described it as a learning opportunity. In the examination, the students were able to integrate theory with practice, and earlier established knowledge was scrutinised when reflecting on the scenarios. The examination added aspects to the students' learning that prepared them for the real world of nursing in a safe environment without risking patient safety. The study findings suggest that examinations in clinical simulation laboratories can be a useful teaching strategy in nursing education. The use of high-fidelity patient simulators made the examination authentic. The reflections and feedback on the scenario were described as significant for the students' learning. Undergraduate nursing students can improve their knowledge, understanding, competence and skills when such examinations are performed in the manner used in this study.

    Abstract [en]

    OBJECTIVES: The aim of this study was to compare student nurses' communication self-efficacy, empathy, and mindfulness across two countries, and to analyse the relationship between these qualities.

    METHODS: The study had a cross-sectional design. Data was collected from final year student nurses in Norway and Sweden. Communication self-efficacy, empathy, and mindfulness were reported by questionnaires; Clear-cut communication with patients, Jefferson Scale of Empathy, and Langer 14 items mindfulness scale.

    RESULTS: The study included 156 student nurses, 94 (60%) were Swedish. The mean communication self-efficacy score was 119 (95% CI 116-122), empathy score 115 (95% CI 113-117) and mindfulness score 79 (95% CI 78-81). A Mann-Whitney test showed that Swedish students scored significantly higher on communication self-efficacy, empathy, and mindfulness than Norwegian students did. When adjusted for age, gender, and country in a multiple linear regression, mindfulness was the only independent predictor of communication self-efficacy.

    CONCLUSION: The Swedish student nurses in this study scored higher on communication self-efficacy, empathy, and mindfulness than Norwegian students did. Student nurses scoring high on mindfulness rated their communication self-efficacy higher.

    PRACTICE IMPLICATIONS: A mindful learning approach may improve communication self-efficacy and possibly the effect of communication skills training.

    Under senare år har den perioperativa sjuksköterskegruppen alltmer uppmärksammat bristerna i den del av sjuksköterskans omvårdnadsdokumentation som varit direkt kopplad till den perioperativa omvårdnaden. Omvårdnadsdokumentationen har ofta varit spridd på många ställen i olika journalsystem. Den vårdvetenskapliga problematiken ligger i att den perioperativa sjuksköterskan i mötet med patienten tenderat att mer dokumentera med fokus på den rent medicinska omvårdnadsdelen jämfört med den omvårdnadsdokumentation som är relaterat till patientens lidande/välbefinnande. Detta kan ge kvalitativt sämre förutsättningar för en god intraoperativ vårdrelation nästa gång patienten möter sjuksköterskan på en operationsavdelning. Förståelsen av de erfarenheter patienten gjort och som kunnat påverka hela patientupplevelsen har i och med detta hamnat i bakgrunden. Det har resulterat i att anestesi- och operationssjuksköterskan inte fullt ut tagit vara på kunskapen i patientens upplevelse av tidigare operationstillfälle. 3 Syftet med uppsatsen är att belysa anestesi- och operationssjuksköterskors omvårdnadsdokumentation av den perioperativa processen Metoden är empirisk studie i form av ett frågeformulär riktad till 6 operationsenheter i västra Sverige. Resultatet av studien visar att hälften av enheterna inte dokumenterar patientens upplevelser av operationstillfällen, upp till en fjärdedel av den totala omvårdnadsdokumentationen är relaterat till patientens lidande/välbefinnande medan resten är relaterad till medicinteknisk dokumentation. Studien visar att 5 av de 6 enheterna använder sig samma dokumentationsmall. Studien visar också att det finns ett behov av nationella gemensamma termer och begrepp inom perioperativ omvårdnadsdokumentation.

    Abstract [en]

    Historically, the ambulance service has been viewed as an organization responsible for the transportation of sick people into hospital, but as a result of advances in surgical techniques, resuscitation and pharmacology, this has led to the discipline of pre-hospital emergency care becoming established and recognized as an important part of the patient's total care. In addition, medical and technical developments have led to pre-hospital emergency care becoming a kind of advanced nursing and an important first link in the chain of care. This has resulted in the emergence of a new discipline of ambulance nurses. Their entrance onto the scene is gradually changing and developing the ambulance service. Nurses with a background in anaesthetics, intensive care and cardiology units have for many years been employed for pre-hospital work (Suserud et al. 1998). In Sweden, nurses, and in particular anaesthetic nurses, have been increasingly used for pre-hospital work in Mobile Intensive Care Units (MICU) and on medical emergency teams. These nurses have traditionally taken an active part in the intra-hospital routine treatment of patients with complex conditions and who are clinical unstable. Medical directors' and ambulance chiefs' views have been that those nurses who are used to working independently and have competence in advanced life support are suitable for this role. I also think that many of them, as well as many ambulance nurses, are attracted to working at the front line. However, I would say that most are not prepared for the diversity of patient they will meet outside the acute hospital.
