    Abstract [en]

    The first aim of this study was to investigate how long-term mentally ill persons adjusted to life after de-institutionalisation. The second aim was to estimate the relevance of interpretations made previously in a qualitative study, concerning long term mentally ill person’s incapability to benefit from new experiences. A survey was conducted in 1996 and a follow- up was made in 1998. A randomly chosen sample of longterm mentally ill persons replied to questions about their living conditions as well as interpersonal relations, personal experiences and self-esteem. Most of the participants in the study were living alone and their everyday lives seemed lonely and inactive. In spite of decreased psychiatric consultations after the psychiatric de-institutionalisation, they changed neither their external nor their internal world during the two years covered by this study.

    Abstract [en]

    The aim of this case study has been to analyze factors that hinder the provision of adequate nursing care in one emergency care unit (ECU). The research question was, What are the conditions that explain the provision of inadequate nursing care in an ECU with reported problems concerning caring attitudes? The design of the study was exploratory and interpretive within a hermeneutic tradition. Nurses were interviewed, and field notes were made from participant observations of routine work and staff meetings. An interpreted whole revealed a lack of a holistic perspective. The difficulty of these nurses in accomplishing a holistic perspective seemed to be due to overvaluation of measurable tasks, such as medical interventions, and undervaluation of nonmeasurable tasks, such as caring. As a consequence, caring interventions failed to strengthen the nurses’ self-esteem in the same high degree as medical tasks.

    Abstract [en]

    In this study eleven items concerning wants and needs of severely mentally ill persons were compared with estimations of the same items made by professional caregivers in psychiatric care and social welfare services. A questionnaire with ten-point Likert scales was distributed to 205 participants, 65 professionals within psychiatric care, 97 professionals within social welfare services and 43 severely mentally ill persons. The Likert scales dealt with living conditions, daily activities, nursing interventions and social welfare services. The severely mentally ill persons scored their wants and needs and professional caregivers estimated the importance of every item concerning the same issues. Professional caregivers made quite similar estimations irrespective of their professional or organizational belonging. They did, however, differ quite a lot from the wants and needs made by the severe mentally ill persons themselves.

    Abstract [en]

    The aim of this study was to analyse and describe non-caring encounters at an emergency care unit (ECU). The research approach was life-world hermeneutics, and the research question was: what are the conditions leading to non-caring encounters at an ECU? Nine nurses and nine patients were interviewed. The interpretative analysis reveals an adaptation to organisational demands for efficiency, on the part of both nurses and patients. This form of adaptation seems to constitute a precondition for a well-functioning ECU. Furthermore, a comparison with a study of the intersubjective aspects of caring for aggressive patients reveals unexpected similarities: the carers are absent in both contexts. In nursing education and ECU-practice caring competence must thus include capacity to be present when patients express their needs worries and questions about care.

    Abstract [en]

    Today mental health professionals are challenged in supporting people with severe mental illness that live within their communities. The community treatment is, however, characterized by an uncertainty about how to best support them in their everyday lives, and professionals from different disciplines often have divergent opinions about the care. The aim of this study is to explicate the existential meaning of living with severe mental illness. Interviews with persons who relocated from an institutional setting to a community placement were analyzed within an interpretive approach. The results of the study found that people with severe mental illness experience an existential loneliness due to difficulties in changing previous suppositions about human relationships. They do not develop connections through shared new experiences with other people in their lives. One central implication of the findings is that because people with severe mental illness seem unable to benefit from new experiences, mental health nurses should consider relational aspects when planning, implementing, and evaluating nursing care.

    This study focuses on the views of professional health care providers within a geographical district in Sweden. A total of 52 interviews with persons representing different professional levels within psychiatry and the social services were conducted. The purpose was to compare their understanding of what their responsibility entailed as well as their perceptions of goals and required competence. A content analysis, supplemented with interpretations within a hermeneutic tradition, showed that there are both similarities and differences within and between the two organizations. A common finding is that professionals with the least education are to a great extent responsible for the practical care of persons with long-term mental illness. The implication of this finding is that it is important to be aware of this division of responsibility when developing psychiatric nursing and the home care services according to the needs of long-term mentally ill patients.

    Abstract [en]

    The aim of this study is to analyze and describe lived experiences of being a father of an adult child with schizophrenia. Interpretations of interviews with seven Swedish fathers of sons or daughters with schizophrenia revealed a pattern of gradually changing existential consequences. After an initial period of shock when receiving the diagnosis, a long struggle to regain control follows. The findings are presented in a structure based on eight different aspects of this struggle, which seems to be characterized by a balance between grieving and adaptation. An important conclusion is that the fathers' life-world must be attended to in professional family interventions.

    Abstract [en]

    This deliverable is the first in the work package to report on the results of the assessment. Since the start the work in WP7 was done in close collaboration with the “Dissemination” and “Technology transfer”  work packages (WP8 and 9) and is a part of the knowledge transfer activities formed by these three  work packages. 

    The deliverable also provides a training plan for the years 2015 and 2016. The implementation of this  plan  has  already  started  with  a  first  training  workshop  on  automated  metadata  extraction  and  encapsulation during the IDCC conference 2015. As the training plan (see section 5) takes a central  role  in outlining  the path  for  the upcoming WP activities  it has been  formulated  in a way  to allow  readability even in isolation from the rest of this document.

    Abstract [en]

    The objective of this document is to provide background information relating to the production of training material and the training page hosted as part of the PERICLES website. The rationale behind this process is also described and justified.

    The production of the training material is based on aligning the outputs of the project’s research with a user-friendly design. This process helps to determine the topics and types of material to be created and collected that will most efficiently and appropriately serve the needs and abilities of the target groups. This involves an analysis of the target group that might be both interested in and have the necessary background to understand the research topics and tools developed and examined during the project. Other considerations include the way the material could be used and how the use can be supported through the look and feel of the material.

    The design of our modular training page is based on a format that has been proven successful in other settings (e.g. at the Swedish National Agency for Education). Within this online training package each module is self-explanatory and can be used without the need for additional input or explanation from the content authors. The information provided within each module helps the user to identify whether the module and its contents are of relevance. The underlying structure also helps the authors to cover the topics that need to be covered and provide the content in a way that will fit with the pedagogical ideas embedded in the design of the package.

    The choice of material and format used in the training material is strongly linked to the nature of the topic, be it demonstrations, screencasts and user guidelines for tools, presentations and short texts for more overview and introductory material, or articles and videos for more theoretical topics requiring a more descriptive, narrative type of communication.

    The creation of modules is a work-in-progress, as many of the results are still to be finalised. Currently, we have completed two modules that are representative of the research and development outcomes.

    - “Dynamics of knowledge organisation” studies the involvement and application of semantics in Digital Preservation and focuses on two different but potentially interrelated approaches: (a) vectors, matrices, and vector fields; (b) graph-based structures and ontologies. This module clearly belongs in the category of theoretical research; therefore, a lot of effort has been made to communicate this in an attractive way, with webcast seminars for example.

    - “Sheer curation” is a good example of a technical development outcome. The module provides the context that leads to demonstrating the tools (PET and PeriCAT). This context has been presented in a way that can be used without the tools, as it provides high-level introductions to related topics, state of the art in the related research and practice, and overviews of current technologies etc.

    Other modules are in development on topics including Appraisal; Digital Ecosystem Models; Recent Developments in the Conservation of Digital Art; Policies & Policy Creation and management using the Policy Editor; Earth and Space Data for the Future; and Modelling and Planning Repository systems.

    This collection of modules forms a repository of useful learning material for students and trainers and fosters long-term use of PERICLES results for training purposes in a self-learning environment. In parallel to this, other suitable materials are presented in tutorials and in current and upcoming academic programmes as exemplified below.

    Background:  Becoming a parent is overwhelming for most men and women and alters the sexual relationship for many couples. Aim:  To describe fathers’ experience about sexual life after childbirth within the first 6 months after childbirth. Method:  A descriptive design, using content analysis with a qualitative approach, based on focus group discussions and one-to-one interviews. Participants:  Eight first-time and two subsequent fathers participated. Results:  Three subthemes were identified: Struggling between stereotypes and personal perceptions of male sexuality during transition to fatherhood; new frames for negotiating sex; a need to feel safe and at ease in the new family situation. The overarching theme emerged as ‘transition to fatherhood brings sexual life to a crossroads’ and guided us to a deeper understanding of the difficulties men experience during the transition to fatherhood. To get sexual life working, a number of issues had to be resolved, such as getting involved in the care of the baby and the household and getting in tune with their partners in regard to sexual desire. The men needed to be reassured and prepared for this new situation by health care professionals. Conclusions:  New fathers in our study put the baby in focus in early parenthood and were prepared to postpone sex until both parties were ready, although they needed reassurance to feel at ease with the new family situation. The fathers’ perceptions of sexual life extended to include all kinds of closeness and touching, and it deviated from the stereotype of male sexuality. This is important information for health care providers and midwives to be aware of for their encounters with men (and women) during the transition to fatherhood, and parenthood and can contribute to caring science with a gender perspective on adjustment of sexual life after childbirth.

    In this article, we discuss important aspects of the perceived problematic relationship between self-management and standardization. The article presents data from three case studies con - ducted within manufacturing companies in Sweden, where the popularity of lean production has led to a renaissance for short-cycle and standardized assembly work in settings that traditionally have made use of sociotechnical production design. The data suggest that the implementation has not contributed to an increased commitment, smooth operations, and capacity for change and innovation. Despite these not so positive results, it is argued that it is possible to combine self-management principles with lean production and standardization if 1) the implementation of lean is done with a contextual sensitivity, 2) a balance is reached between the use of standards on the one hand and work enrichment on the other, and 3) a feeling of ownership as regards both implementation and production process is upheld among the production personnel.

    Abstract [en]

    As computational technology and new materials are entering the world of textiles, our view on the textile material is challenged; the areas of textile design and interaction design start to merge. Designing interactive textiles means communicating new expressional forms through textile material. This thesis aim to contribute to the exploration of designing interactive knitted textiles with focus on linking interactive properties, especially visual and tactile, to different ways of expressing them. Design examples and experiments presented are meant to show new possibilities for designing textile material able to change structure, colour and temperature in different ways in response to environmental stimuli. As a way of presenting, and reflecting on, these interactive properties, a notion of expression diagrams is introduced.

    Abstract [en]

    Purpose: The purpose of this study is to elucidate social and material factors that contribute to shape the design and use of a digital (i.e. web-based) service platform, which is aimed at facilitating service innovation in a collaborative setting of service providers and customers.

    Design/methodology/approach: Empirical data was produced in connection to interventions including one of the authors collaborating with service providers and customers using the platform. Two sessions taking place in workplaces were filmed, photographed, and recorded. Participants in these sessions were also responding to questions from the researcher who took field notes. A practice-theoretical approach was applied in order to capture and analyze practices around the design, implementation, and use of the platform.

    Findings: Three prominent themes were discerned in the empirical material: material objects; workplace routines including “distant others”; and sociality. Material objects function as intervening actors and thereby contribute to shape practice. Workplace routines as well as physically absent colleagues and contacts become present through conversations around the platform. Sociality refers to the observation that participants are engaged in interactive practices that contribute to shape how the digital service platform is designed, implemented and used.

    Originality/value: The practice-based approach offers a deepened understanding of how sociomaterial context matters for the design and implementation of a digital service platform. On the basis of a sophisticated explication of the notion of context and by employing the notion of absent presence, this study sheds light on factors that often tend to be neglected.

    I vårt samhälle lägger vi stor vikt vid möjligheten att själva få välja det liv vi ska leva, inom de ramar som lagstiftning, ekonomi och hänsynen till andra människor tillåter. En viktig anledning till att vi värdesätter detta är att vi har olika uppfattningar om vad som är ett gott liv, beroende på sådant som bakgrund, uppfostran, religiös och kulturell uppfattning men kanske framförallt beroende på de egna överväganden vi gör över vad som är värdefullt i livet. När vi är yngre är detta självklart för de flesta, vi har olika musikstil, olika klädstil, lever i olika former av relationer, uppskattar olika former av nöjen, väljer olika former av arbete och har olika inställning i politiska, religiösa och andra livsfrågor. Samtidigt finns det en risk att när vi blir äldre, så blir vi mer och mer betraktade som en stor massa där vi alla har likadana uppfattningar och önskningar om hur livet ska vara. Men det finns egentligen ingen anledning att tro att det bland äldre skulle finnas en mindre variation i sätt att vilja leva sitt liv, utan även då är det väsentligt att ha möjlighet att välja det liv man vill leva.

    Många människor har uppfattningen att när en äldre människa dör så är det inte en lika stor tragedi som när en ung människa dör. Likaså förknippar vi ofta åldrande med död och vi säger sådant som att ’det var ju inte så konstigt att hon dog – hon var ju så gammal’. Utgående från detta kan man ställa sig frågan om hur man ska se på förhållandet mellan åldrande och död, och framförallt hur man ska se på förhållandet mellan en god död och åldrandet. Innan vi tar upp några konkreta aspekter av detta så behöver vi reda ut begreppen kring den goda döden. Med en god död kan vi mena åtminstone fyra olika saker: ett gott döende (dvs. att tiden fram till döden är så bra som möjligt), ett gott dödstillfälle (att själva dödsögonblicket är så bra som möjligt), en god död efter döden (dvs. om det kan hända något efter döden som kan påverka mitt liv) samt om det faktum att jag dör vid en viss tidpunkt är bra eller dåligt. I detta sammanhang ska vi framförallt beröra den första och sista frågan. Och vi börjar med den sista frågan.

    När människor blir äldre och drabbas av sjukdom så ökar deras beroende av andra människors hjälp, i förlängningen innebär det att deras beroende av professionell hjälp ökar och att de därmed blir beroende av äldreomsorg i olika former. Att de blir beroende sker ju inte över en natt utan det sker gradvis och det innebär då att det sker en gradvis förändring av deras normala liv. I detta normala liv försöker de flesta människor skapa ett så gott liv som möjligt för sig själv och sina närmaste och följaktligen bör då även äldreomsorgen ha som mål att skapa möjlighet för den äldre och dennes närmaste att leva ett så gott liv som möjligt, trots sjukdom och ökat beroende (Brülde 2001). Äldreomsorgens mål kan alltså inte i första hand vara att bota utan framförallt att ge möjlighet till ett fortsatt gott liv.

    Within the western health-care context autonomyis a central value. Still, as it is used withinthis context it is far from clear what we areactually talking about. In this article theauthor outlines four different uses or aspectsof autonomy: self-determination, freedom,desire-fulfilment and independence. Oneimportant conclusion will be that in order tobe able to respect autonomy in a way thatactually brings value to the patient’s life weneed to clearly assess what aspect of autonomythe patient values and for what reason it isvalued by the patient.

    In the article the concept of natural death as used in end-of-life decision contexts is explored. Reviewing some recent empirical studies on end-of-life decisionmaking, it is argued that the concept of natural death should not be used as an action-guiding concept in end-of-life decisions both for being too imprecise and descriptively open in its current use but mainly since it appears to be superfluous to the kind of considerations that are really at stake in these situations. Considerations in terms of the quality of life cost of the intervention in relation to the quality and length of life benefits of the same intervention. In referring to the concept of natural death we risk to blur these considerations and end up in difficult distinctions between what is a natural and non- or un-natural death, a distinction which it is argued is of no real moral interest.

    n discussions on palliative care and in palliative care we often hear references to the idea of a ‘death with dignity’ or a ‘dignified death’ in different forms. Dignity is obviously one of the more central concepts in discussions on the topic of a good death, and it is frequently used to label good dying and death, and good care for dying people.† Still, a problem in relation to the concept of dignity is that it is used in a number of different ways and it is far from clear what it implies to talk in terms of dignity in relation to death and dying. In Saunders & Baines (1983) it is even argued that this lack of clarity should result in us not using the concept. In this short paper I will outline and develop some ideas concerning this developed in my thesis A good death. On the value of death and dying (Sandman, 2001). First I will present a number of different uses and connotations of the term ‘dignity’ found in the palliative (and other) care contexts. Then I will focus specifically on the idea of human dignity and its relevance to palliative care. The first part of that discussion will be devoted to the basis of such a human dignity and the second part to the implications that accepting such a dignity will have for palliative care.

    In patient centred care, shared decision making is a central feature and widely referred to as a norm for patient centred medical consultation. However, it is far from clear how to distinguish SDM from standard models and ideals for medical decision making, such as paternalism and patient choice, and e.g., whether paternalism and patient choice can involve a greater degree of the sort of sharing involved in SDM and still retain their essential features. In the article, different versions of SDM are explored, versions compatible with paternalism and patient choice as well as versions that go beyond these traditional decision making models. Whenever SDM is discussed or introduced it is of importance to be clear over which of these different versions are being pursued, since they connect to basic values and ideals of health care in different ways. It is further argued that we have reason to pursue versions of SDM involving, what is called, a high level dynamics in medical decision-making. This leaves four alternative models to choose between depending on how we balance between the values of patient best interest, patient autonomy, and an effective decision in terms of patient compliance or adherence: Shared Rational Deliberative Patient Choice, Shared Rational Deliberative Paternalism, Shared Rational Deliberative Joint Decision, and Professionally Driven Best Interest Compromise. In relation to these models it is argued that we ideally should use the Shared Rational Deliberative Joint Decision model. However, when the patient and professional fail to reach consensus we will have reason to pursue the Professionally Driven Best Interest Compromise model since this will best harmonise between the different values at stake: patient best interest, patient autonomy, patient adherence and a continued care relationship.

    Abstract [en]

    This article analyses and presents a survey of ethical conflicts in prehospital emergency care. The results are based on six focus group interviews with 29 registered nurses and paramedics working in prehospital emergency care at three different locations: a small town, a part of a major city and a sparsely populated area. Ethical conflict was found to arise in 10 different nodes of conflict: the patient/carer relationship, the patient’s selfdetermination, the patient’s best interest, the carer’s professional ideals, the carer’s professional role and self-identity, significant others and bystanders, other care professionals, organizational structure and resource management, societal ideals, and other professionals. It is often argued that prehospital care is unique in comparison with other forms of care. However, in this article we do not find support for the idea that ethical conflicts occurring in prehospital care are unique, even if some may be more common in this context.

    Intermediation in Innovation Communities: the case of Centiro2010Konferensbidrag (Refereegranskat)
    Abstract [en]

    The aim of the paper is to advance our understanding of the growing role of innovation communities, in particular the role of intermediaries in such communities, and also to look for new ways of learning in innovation communities using a new meeting format. The paper provides a literature survey on innovation communities, with an emphasis on intermediaries and their functions. A new form of meeting format called the ”systemic meeting” is used to understand and enhance the knowledge and learning processes in the innovation community of Centiro, a rapidly growing Borås company specializing in IS/IT solutions for demand chain management. There is a wealth of knowledge available in different innovation communities, not the least in the global IS/IT community. But there is a dire need to develop the learning processes in order to get maximum benefit of that knowledge. Systemic meetings have shown to be powerful tools to surface the knowledge-learning interrelationship and to bridge the gap knowledge and learning in innovation communities. The paper focuses on a systemic meeting hold as a part of a Logistics Change Management class at the University of Borås. However, building on that it also provides reflections on how systemic meetings can be used as a tool for intermediaries, or for companies who want to develop their intermediating position as a way to bridge the knowledge/learning gap. The paper will deepen our understanding of how systemic meetings can be used to develop innovation communities.

    Network based intermediation in logistics innovation communities2010Konferensbidrag (Refereegranskat)
    Abstract [en]

    The aim of the paper to investigate how a network organization; in this case by the name of Sweden Logistics, working primarily in the area around the city of Borås in Sweden, can develop its role as an intermediary in a logistics innovation community. The board of the network organization has taken several intermediating initiatives. Two of them are described in this paper. We refer to the first as a pooled initiative, and to the second as a systemic initiative. The first was pooling project resources in order to develop certain common logistics solutions in the area of distance trade return logistics. The second was aiming for a more long term and open distance trade innovation community, centering though in the first step on information support systems for distance trade. With its base in systemic theory and network theory, the paper starts up by a literature review distilling the roles of intermediaries in logistic and other types of innovation communities. On the methodological side the paper investigates the possible roles of systemic meetings as a tool for innovation community organizing. Systemic meetings were used in both of the initiatives. In the first initiative a systemic meeting proved useful for providing consumer insights and for developing ideas on how to shape return logistics in distance trade. In the second initiative a systemic meeting was used to recognize patterns in the existing distance trade innovation community, this in order to find ways to strengthen and develop this for the area important community. The second systemic meeting proved to be more problematic and the paper therefore focuses on a discussion on the possible uses of systemic meetings as a tool to develop long term and open innovation communities.
