Abstract [sv]

Homosexuella, bisexuella, transpersoner, queerpersoner och intersexpersoner [HBTQI] tillhör en minoritet och gruppen erfar diskriminering i samhällets alla nivåer, vården är ej ett undantag. Populationen har sämre hälsa än befolkningen i övrigt och gruppen tenderar att söka vård med lägre frekvens än annan population. Barnmorskan skall ha ett etiskt förhållningssätt och vårda på ett normmedvetet vis, mänskliga värden skall värnas och den vårdsökande skall stödjas i dennes självbild. Dock visar forskning att barnmorskans vårdande som ofta är sprungen ur ett heteronormativt perspektiv resulterar i att HBTQI-personer erhåller sämre vård än befolkningen i övrigt och deras hälsa påverkas negativt av detta. Mer forskning behövs utifrån HBTQI-personers perspektiv på vilka aspekter som bidrar till att mötet med barnmorskan skall upplevas stärkande. Syftet: Att beskriva HBTQI-personers erfarenheter av hur barnmorskans vårdande kan främja hälsa och välbefinnande. Metod: kvalitativ innehållsanalys med induktiv ansats. Datainsamling: Nio semistrukturerade intervjuer som annonserades via social media och hölls via Teams/Zoom. Resultat: Bemötandet som HBTQI-personer erfar i mötet med barnmorskan kan främja hälsa och välbefinnande. Önskvärda egenskaper hos barnmorskan för att mötet med denne skall bli en stärkande erfarenhet är att hen utstrålar en öppenhet och acceptans samt besitter kunskap som bidrar till att personcentrerad vård kan tillhandahållas. Slutsats: Huruvida mötet med barnmorskan främjar hälsa och välbefinnande eller ej är i stort avhängigt på den enskilda barnmorskan bemötande. Goda erfarenheter med barnmorskor bidrar till en ökad kontinuitet och tröskeln till att söka vård i framtiden blir lägre. Då HBTQI-personer lider av ohälsa i högre utsträckning än befolkningen i övrigt så anses det angeläget att främja vårdfrekvensen hos gruppen. Med djupare kunskap kring HBTQI samt de utmaningar som gruppen upplever, skulle barnmorskan aktivt kunna anpassa sitt bemötande och således bedriva personcentrerad vård för att främja välbefinnande och hälsa.

Abstract [en]

Lesbian, gay, bisexual, transgender, queer and intersex people [LGBTQI] belong to a minority and the group experiences discrimination at all levels of society, and healthcare is no exception. The population has poorer health than the general population and the group tends to seek healthcare at a lower frequency than other populations. The midwife should have an ethical approach and provide care in a norm-conscious manner, human values should be protected and the person seeking care should be supported in their self-image. However, research shows that midwife's care, which is often based on a heteronormative perspective, results in LGBTQI people receiving poorer care than the general population and their health is negatively affected by this. More research is needed from the perspective of LGBTQI people on which aspects contribute to the encounter with the midwife being perceived as empowering. Aim: Describe the experiences of LGBTQI people of how the midwife's care can promote health and well-being. Method: Qualitative content analysis with an inductive approach. Data collection: Nine semi-structured interviews that were advertised via social media and held via Teams/Zoom. Results: The treatment that LGBTQI people experience in the meeting with the midwife can promote health and well-being. Desirable qualities in the midwife for the meeting with her to be a strengthening experience are that she radiates openness and acceptance and possesses knowledge that contributes to the provision of person-centered care. Conclusion: Whether the meeting with the midwife promotes health and well-being or not is dependent on the individual midwife's treatment. Good experiences with midwives contribute to increased continuity and the threshold for seeking care in the future is lower. Since LGBTQI people suffer from ill health to a higher extent than the general population, it is considered important to promote the frequency of care in the group. With deeper knowledge about LGBTQI and the challenges that the group experiences, the midwife would be able to actively adapt her approach and thus provide person-centered care to promote well-being and health.
