Kommunal hälso- och sjukvård är en komplex organisation där patientsäkerheten påverkas av de beslut som tas på politisk nivå. Att patienten i den kommunala hälso- och sjukvården vårdas under två lagar och att patientsäkerhet som begrepp inte är lika tydligt i dem båda gör att arbetet blir komplext. Utifrån de svårigheter som finns gällande att bibehålla en säker vård i alla led har Socialstyrelsen tagit fram en nationell handlingsplan för all hälso- och sjukvård med syfte att ge stöd och struktur i patientsäkerhetsarbetet. Dock används inte handlingsplanen fullt ut i kommunal hälso- och sjukvård enligt de undersökningar som gjorts av Socialstyrelsen vilket gjort att frågan ställts varför, och mynnat ut i studiens syfte att öka förståelsen för hur kommunal hälso- och sjukvård arbetar med patientsäkerhet och vad som påverkar deras arbetssätt. Studiens design är en kvalitativ intervjustudie med semistrukturerade frågor som analyserats med hjälp av en innehållsanalys. Dataanalysen har mynnat ut i tre rubriker som besvarade studiens forskningsfrågor. De var, arbetssätt, tolkning samt hinder och förutsättningar. Resultatet visade att arbetet med patientsäkerhet såg olika ut beroende på vem som hade ansvaret och ledde arbetet, begreppet tolkades på olika sätt och arbetet med patientsäkerhet blev personbundet och påverkades av tidigare kunskap och eget intresse i patientsäkerhetsfrågor. Resultatdiskussionen visade konsekvenserna av hur den kommunala hälso- och sjukvårdens arbete påverkade patientsäkerheten i region Jönköpings län. Slutsatsen i studien blev att bristande styrning och ledning var den främsta orsaken till att ett fullgott patientsäkerhetsarbete inte kunde bedrivas. 

Municipal healthcare is a complex organization where patient safety is influenced by decisions made at the political level. The fact that patients in municipal healthcare are treated under two laws, and that the concept of patient safety is not equally clear in both, makes the work complex. Given the difficulties in maintaining safe care at all levels, the National Board of Health and Welfare has developed a national action plan for all healthcare with the aim of providing support and structure in patient safety work. However, the action plan is not fully utilized in municipal healthcare according to surveys conducted by the National Board of Health and Welfare, which has raised the question of why. This has led to the study's aim of increasing understanding of how municipal healthcare works with patient safety and what influences their work methods. The study's design is a qualitative interview study with semi-structured questions analyzed through content analysis. The data analysis resulted in three themes that answered the study's research questions: work methods, interpretation, and barriers and conditions. The results showed that patient safety work varied depending on who was responsible and led the work, the concept was interpreted in different ways, and patient safety work became person-dependent and influenced by previous knowledge and personal interest in patient safety issues. The discussion of results showed the consequences of how the work in municipal healthcare affected patient safety in the Jönköping County region. The study concluded that the main reason for the inability to conduct satisfactory patient safety work was a lack of governance and leadership.
