Palliativ vård är vanligt förekommande och i denna vårdform finns en etisk komponent som regelbundet ger upphov till problem och dilemman för sjuksköterskor som arbetar patientnära. Orsakerna till dessa etiska svårigheter är mångfaldiga samt komplexa där sjuksköterskor behöver bemöta dessa med kompetens och mod för att inte vårdkvaliteten ska påverkas negativt. Etiska problem kan i värsta fall leda till att sjuksköterskor blir utbrända och att patienter orsakas ett lidande när vården brister. Syftet med litteraturstudien är att beskriva sjuksköterskans upplevelser av etiska dilemman inom palliativ vård. En litteraturöversikt används som metod och studien bygger på kvalitativ data inhämtad från databaser med omvårdnadsfokus där nio kvalitativa artiklar utgör resultatet. Litteraturstudiens resultatet innefattar huvudsakligen teamets olikheter utifrån sjuksköterskans agerande och hur väl samarbetet med kollegor fungerar, den lidande patienten med betoning på vårdrelationen och närståendes behov samt organisationens inflytande där resurser och olika begränsningar har inverkan på upplevelserna. Slutsatserna är att sjuksköterskan möter flertalet olika etiska problem och svåra val där upplevelserna av dessa påverkas av exempelvis moral, personliga värderingar, erfarenhet samt vårdlagets samarbete. Etiska problem och dilemman har, om de inte hanteras väl av sjuksköterskan, i värsta fall potential att skada vårdrelationen med patienten och närstående, bidra till ökat lidande för patienten och leda till en otrygg situation för alla inblandade i den palliativa vården. Sjuksköterskans etiska medvetenhet och kompetens är essentiell för att patienten ska erhålla en god palliativ vård utifrån de fyra palliativa hörnstenarna och vårdmodellen De 6 S:en.

Palliative care is common and in this form of care there is an ethical component that regularly gives rise to problems and dilemmas for nurses who work close to patients. The reasons for these ethical difficulties are multiple and complex, where nurses need to respond to them with competence and courage so that the quality of care is not negatively affected. Ethical problems can, in the worst case, lead to nurses becoming burned out and patients suffering when care is lacking. The purpose of the literature study is to describe the nurse's experiences of ethical dilemmas in palliative care. A literature review is used as a method and the study is based on qualitative data obtained from databases with a nursing focus where nine qualitative articles form the result. The result of the literature study mainly includes the team's differences based on the nurse's actions and how well the collaboration with colleagues works, the suffering patient with an emphasis on the care relationship and the needs of relatives and the influence of the organization where resources and various limitations have an impact on the experiences. The conclusions are that the nurse faces several different ethical problems and difficult choices where the experiences of these are influenced by, for example, morals, personal values, experience and the cooperation of the care team. Ethical problems and dilemmas, if they are not handled well by the nurse, in the worst case have the potential to damage the care relationship with the patient and relatives, contribute to increased suffering for the patient and lead to an unsafe situation for everyone involved in palliative care. The nurse's ethical awareness and competence is essential for the patient to receive good palliative care based on the four palliative cornerstones and the 6 S care model.
