Abstract [sv]

Bakgrund: Jones Dependency Tool (JDT) är ett verktyg som används på akutmottagningar i England och Australien för att identifiera vårdbehov hos vårdsökande personer. I Sverige finns inget instrument som bedömer vårdbehov hos vårdsökande personer, trots att vården till större grad blir mer komplex på akutmottagningarna i Sverige.

Syfte: Att översätta, kulturellt adaptera, samt validitets/reablilitets testa Jones Dependency Tool till en svensk kontext.

Design: Convergent mixad metod

Metod: Genom gruppintervjuer med efterföljande pilot testning på fiktiva fall inhämtade från en nationell triageutbildning skapa kulturell samstämmighet i användandet av Jones Dependency Tool.

Resultat: Intervjuer med tre grupper (expertpanel 1 och expertpanel 2) samt en uppföljande grupp (operativ utvärderingspanel) skapade en validitets samstämmighet; Content Validity Ratio (0,92) med efterföljande pilottestning n=16 på vardera tio fall (totalt 160 mätningar) som visade på en hög reliabilitet med en cronbachs a på 0,82. En differentierad faktoranalys med tydligt indelade grupper med liten korrelation. Betydelsefulla faktorer var verktyget i sig, vilket patientfall som bedömdes samt vårdarens bakgrund och erfarenhet. Användarvänligheten mättes genom System Usability Score som visade på ett knappt godkänt värde (53,4).

Konklusion: Jones Dependency Tool har översatts och kulturellt adapterats med god kvalitet, med goda psykometriska värden, och bedöms kunna införas i en svensk kontext. Implementering utan föreslagna interventioner bör undvikas, då enbart mätning utan åtgärder har en minimal effekt på den direkta vården.

Inverkan: Vid implementering av JDT ska lokala interventioner identifieras och appliceras i relation till totalpoängen som instrumentet ger för att öka patientsäkerheten. Interventionerna måste identifiera på lokal nivå eftersom det finns olika organisatoriska förutsättningar vid varje akutmottagning. Vidare forskning skulle kunna identifiera dessa på nationell nivå, för att ytterligare stärka rätten till lika vård.

Abstract [en]

Background: The Jones Dependency Tool (JDT) is a tool used in emergency departments in England and Australia to identify care needs in care seekers. In Sweden, there is no instrument that assesses the care needs of people seeking care, despite the fact that care is becoming more complex in emergency departments in Sweden.

Purpose: To translate, culturally adapt, and validity/reablilitets test the Jones Dependency Tool into a Swedish context.

Design: Convergent mixed method

Method: Through group interviews with subsequent pilot testing on fictitious cases obtained from a national triage training, create cultural coherence in the use of the Jones Dependency Tool.

Results: Interviews with three groups (expert panel 1 and expert panel 2) and a follow-up group (operational evaluation panel) created a consensus of validity; Content Validity Ratio (0.92) with subsequent pilot testing n=16 on each of ten cases (160 measurements in total) showing a high reliability with a cronbach a of 0.82. A differentiated factor analysis with clearly divided groups with little correlation. Significant factors were the tool itself, which patient case was assessed and the carer's background and experience. User-friendliness was measured through the System Usability Score, which showed a barely passed value (53.4).

Conclusion: The Jones Dependency Tool has been translated and culturally adapted with good quality, with good psychometric values, and is considered to be able to be introduced in a Swedish context. Implementation without proposed interventions should be avoided, as only measurement without measures has a minimal effect on direct care.

Impact: When implementing JDT, local interventions should be identified and applied in relation to the overall score provided by the instrument to increase patient safety. The interventions must be identified at the local level because there are different organizational conditions at each emergency department. Further research could identify these at national level, to further strengthen the right to equal care.
