Abstract [sv]

Bakgrund: Arbetet i glesbygdsambulans innefattar långa avstånd och ibland brist på assisterande resurser. Ambulanspersonalen måste fatta snabba beslut på egen hand och vårda kritiska patienter under lång tid till akutsjukhus. Syfte: Att beskriva sjuksköterskors upplevelser som ensam vårdare av kritiskt sjuka patienter under långa ambulanstransporter i glesbygd. Metod: 15 sjuksköterskor som arbetar i glesbygdsambulans intervjuades med semi-strukturerade frågor. Det insamlade materialet bearbetades med utgångspunkt i kvalitativ innehållsanalys.

Analysen av materialet resulterade i två kategorier och sex underkategorier. Arbete i glesbygd innebär utmaningar - Resursbrist innebär mer än avsaknad av fler händer - Teknikproblem kan försvåra beslutsfattandet - Långa transporter av kritiskt sjuka patienter kräver planering. Sällanhändelser - när det inte går att få rutin - Samarbetet mellan kollegor är mycket betydelsefullt - Förmågan att behålla ett inre lugn i stressade situationer - Utbildning ger ökad trygghet i arbetet. Slutsats: Resultatet visar att det krävs god planering och flexibelt tänkande när en kritisk patient ska vårdas under en lång transport. Arbetet kan upplevas som stressande om inga assisterande resurser finns att tillgå. Trots detta upplever deltagarna ett inre lugn som hjälper dem att fokusera på arbetsuppgiften.

Abstract [en]

Background: The work in sparsely populated areas includes long distances and sometimes a lack of assistive resources. Ambulance nurses must make quick decisions on their own and caring the critical patients for a long time to the emergency hospitals. Aim: The purpose of this study is to describe nurses' experiences as the sole carer with critically ill patients during long ambulance transports in rural areas. Method 15 nurses working in ambulances in sparsely rural areas were interviewed with semi-structured questions. The collected material was processed based on qualitative content analysis.

The analysis of the material resulted in two categories and six subcategories. Working in sparsely populated areas means challenges - Lack of resources means more than the lack of more hands - Technology problems can make decision-making more difficult - Long transports of critically ill patients require planning. Rare events - when it is not possible to get routine - Cooperation between colleagues is very important - The ability to maintain inner peace in stressful situations - Education provides increased security at work. Conclusions: The results show that good planning and flexible thinking are required when a critical patient is to be cared for during a long transport. The work can be perceived as stressful if no assistant resources are available. Despite this, the participants experience an inner calm that helps them focus on the task at work
