Abstract [sv]

I samband med att den psykiska ohälsan i samhället ökar följer ett ökat behov av omhändertagandet av patientgruppen inom ambulanssjukvården. Uppdrag där patienten har tidigare känd psykisk sjukdom beskrivs vara det näst vanligaste. Psykisk sjukdom är en stark riskfaktor för suicid. År 2019 dog 1269 personer av suicid i Sverige och 6802 personer vårdades efter suicidförsök. Syftet med studien var att beskriva ambulanssjuksköterskors upplevelse av att vårda psykiatripatienter med inriktning på den suicidala. En intervjustudie genomfördes med tio informanter, som alla var specialistutbildade sjuksköterskor inom ambulanssjukvård. Intervjuerna analyserades med kvalitativ metod med induktiv ansats.

Resultatet visar att ambulanssjuksköterskorna upplever att de saknar stöd och har otillräcklig kunskap i bedömningen av den suicidala patienten, vilket i vissa fall leder till att bedömningen lutas mot mätvärden och somatiska symtom istället för det psykiska måendet. Att patientgruppen behöver få ta tid och att bemöta dem med ett ärligt och öppet förhållningssätt beskrivs avgörande för hur vården blir. Bättre samverkan med psykiatrin, både gällande läkarstöd och med rådfrågning och information via psykakuten är enligt resultatet önskvärt. Då riktlinjer saknas blir patientbedömningen ojämlik och beroende av den enskilda ambulanssjuksköterskans erfarenhet. 

Abstract [en]

In connection with the increase in mental illness in society, there is an increased need for the care of this patient group in ambulance care. Assignments where the patient has a previously known mental illness are described as the second most common. Mental illness is a strong risk factor for suicide. In 2019, 1269 people died of suicide in Sweden and 6802 people were cared for after suicide attempts. The purpose of the study was todescribe the ambulance nurses' experience of caring for the psychiatric patient with a focus on the suicidal. An interview study was conducted with ten informants, all of whom were specialist nurses in ambulance care. The interviews were analyzed using a qualitative method with an inductive approach.

The results show that the ambulance nurses feel that they lack support and do not have sufficient knowledge in the assessment of the suicidal patient, which in some cases leads to the assessment being tilted towards measured values and somatic symptoms instead of the mental state. The fact that the patient group needs to be allowed to take time and to approach them with an honest and open approach is described decisively for how the care will be. According to the results, better collaboration with psychiatry, both regarding medical support and with consultation and information through the psychiatric emergency room, is desirable. When guidelines are lacking, patient assessment becomes unequal and dependent on the individual ambulance nurse's experience.
