Abstract [sv]

Målet med palliativ vård är att ge personer med icke botbar sjukdom möjlighet till optimal livskvalitet genom att lindra lidande. Smärta är ett vanligt symtom bland personer som vårdas palliativt och personen är ofta otillräckligt smärtlindrad. Detta ställer stora krav på sjuksköterskans kompetens att vårda dessa personer. Syftet var att beskriva sjuksköterskans upplevelse av att vårda personer med smärta i specialiserad palliativ vård. Studien har genomförts som kvalitativ metod med induktiv ansats och bygger på semistrukturreade intervjuer som datainsamlingsmetod. Sjuksköterskor verksamma inom tre olika specialiserade palliativa enheter, avancerad sjukvård i hemmet (ASIH) har intervjuats. Resultatet av studien har visat att sjuksköterskan inom specialiserad palliativ vård hade bra stöd från teamet och samtliga sjuksköterskor upplevde att de hade erfarenhet och tillräcklig kunskap för att vårda personen med smärta i hemmet. När det inte gick att smärtlindra fullt ut kunde sjuksköterskan känna frustration och maktlöshet men med stöd av teamet och med inställningen att alltid försöka lindra lidandet oavsett kunde dessa känslor balanseras. Det framkom även i resultatet att det var goda relationer med personen som vårdades, med deras närstående och med kollegor i teamet som låg till grund för en personcentrerad vård. Genom att undersöka sjuksköterskors upplevelser av att vårda personer med smärta i palliativt skede kunde omvårdnads strategier som är av betydelse identifieras och bidra till framtidens vård. Sjuksköterskor inom specialiserad palliativ vård beskrev sig ha tillräckligt med stöd, tid och resurser för att vårda personer med smärta. De möttes av tacksamhet och upplevde sitt arbete meningsfullt.

Abstract [en]

The goal of palliative care is to give people with the incurable disease the opportunity for optimal quality of life by relieving suffering. Pain is a common symptom among people who are cared for palliatively and the person is often insufficiently relieved. This puts great demands on the nurse's competence to care for these people. The purpose was to describe the nurse's experience of caring for people with pain in specialized palliative care. The study was conducted as a qualitative method with an inductive approach and is based on semi-structured interviews as a data collection method. Nurses working in three different specialized palliative care units, advanced home care (ASIH) have been interviewed. The results of the study have shown that the nurse in specialized palliative care had good support from the team and all nurses felt that they had experience and sufficient knowledge to care for the person with pain at home. When it was not possible to fully relieve pain, the nurses could feel frustration and powerlessness, but with the support of the team and with the attitude to always try to relieve the suffering regardless, these feelings could be balanced. What emerged in the result was those good relationships with the person being cared for, with their relatives, and with colleagues in the team were the basis for a person-centered care. By examining nurses' experiences of caring for people with pain in the palliative care stage, important nursing strategies could be identified and contribute to future care. Nurses in specialized palliative care described themselves as having enough support, time, and resources to care for people with pain. They were met with gratitude and experienced their work meaningfully.
